About Us

New Patient Scheduling:

As a new patient, we allow more time for your visit with your provider. Because our time is as valuable as yours, we ask that you call us 24 hours in advance if you have to reschedule or cancel your appointment. To secure your first appointment, we will request a credit card number to keep on file. A $50.00 charge will be made to your credit card account for appointments that are cancelled or changed in less than 24 hours.

Before your first visit:

  • Verify the provider you are seeing is contracted with your insurance plan.
  • Ask your insurance if you need a referral from your primary care physician.
  • Make payment arrangements if you do not have insurance coverage.
  • Make a list of questions or concerns you wish to discuss with us.

On the day of your appointment, please bring:

  • Your insurance card
  • A picture ID (driver’s license)
  • Completed New Patient forms (see forms section to print)
  • A list of all your current medications
  • A list of questions or concerns you may have
  • Payment for services rendered. We accept checks, cash and credit cards (Visa, Master Card and American Express).

 

Office Visits

Please arrive to the office 15 minutes before your appointment. We understand traffic can slow you down and we do allow a 20 minute “grace” period. If you are late, you may be asked to reschedule your appointment.

Since many of our services are driven by your insurance plan, it is imperative to bring your insurance card with you to your appointment. If you do not have your insurance card, you will be asked to reschedule your appointment or you may have the option of paying for the services at the time of the visit.

We see patients by appointment only unless there is an urgent medical situation. We will work hard to honor your appointment time and intend to keep you informed of any delay. Should your physician be called away for a delivery or medical emergency, you will be offered the option of rescheduling your appointment, waiting to see your physician, or seeing another provider who is available in the office. Please be assured that all of our providers offer the same excellent level of service.

Phone Calls

We receive a large volume of phone calls, especially on Mondays and Fridays. You most likely will reach our voice mail to leave a message. You may leave a voice mail message with our nurse by calling 404-252-3898. Our providers have a full schedule and we try not to interrupt them while they are seeing patients except for emergencies.

Please leave a detailed message of:

  • your name and date of birth
  • your concern
  • your provider
  • pharmacy phone number
  • contact phone number to return your call.

All voice mail boxes are checked regularly and messages are handled during business hours.

Medication Refills

Call our office during business hours and leave a voice mail for medication refills. Your request will be processed within 24 hours or less. Please provide the following information:

  • Patient’s name and date of birth
  • Drug name, dosage and frequency taken
  • Pharmacy name and phone number

Calls received after 4:00 pm M-TH or after 12 noon on Friday will be processed the next business day. Please to not call after hours for routine medication refills.

Test Results

We understand that your test results are very important to you. Please allow at least 5 business days before calling us to check on test results. Some tests take longer than 5 days to be processed and sent back to the office. We ask you to be patient and know that you will be notified once your provider has reviewed your results.

Your provider or their nurse will notify you of your results by phone, or may instruct you to access your test results via computer through Quest Labs.

If you do not hear from us within a reasonable time, please call the office to speak with the nurse.

After Hour Emergencies

Your provider is on call after office hours, weekends and holidays. Occasionally, another provider may be on-call for your provider.

Please limit calls to serious illnesses or if you think you are in labor. You may contact your provider by calling the office phone number 404-252-3898. You will be connected with the answering service for assistance.

Please have a pharmacy phone number available for your provider if a prescription is indicated.

For emergency situations, go immediately to the Emergency Room. Your doctor will be contacted from the Hospital.

Completing Medical Forms

We are happy to complete forms such as marriage license, school/camp, adoptions, disability and FMLA. Due to the time and medical expertise involved, we charge a fee of $25.00 per form which will be collected prior to releasing the form. Please allow us 7 business days to complete your form and ask that you plan accordingly. It will be your responsibility to complete your portion of the form and return it to the appropriate institution.

Confidentiality

We consider your medical information strictly confidential. Your medical records can only be released with your written permission. If you would like your records sent to another physician, we will require a written authorization from you. You may print a Medical Records Release Form from our website for your convenience.

Provider Spotlight

Winifred Lin Soufi, M.D, Ph.D.

Winifred_smallDr. Winifred Soufi joined Women’s Health Associates in 1996. She is board certified and practices obstetrics and gynecology. She is one of the top physicians in her field and also a clinical assisitant professor in the Department of Obstetrics and Gynecology at the Medical College of Georgia. She is on the staff of Northside Hospital.
